Your role
As Postdoc, you become a central part of the project's research group and have the opportunity to work with the entire research process from mapping and development for implementation, evaluation and publication.

Your tasks will be:

  • map existing patient and transition courses and practice for identifying and handling support needs
  • plan and implement data collection among citizens, patients, health professionals and organizational actors
  • apply and combine relevant qualitative and quantitative methods
  • contribute to the development and operationalization of the Transition Support model, including the strategy of support needs and development of support profiles
  • enter into co-design with citizens, clinicians and municipal employees
  • plan and implement implementation of the model in selected patient progress
  • contribute to the evaluation of the usability of the model, implementation, importance to citizens and resource consequences
  • analyze and publish results in international peer-reviewed journals as well as conveying relevant relevant partners.

The project consists of three coherent phases: mapping, development and co-design and implementation and evaluation.

We imagine that you
Has a PhD degree within a relevant area and experience of independent research and scientific publishing. Your background can, for example, Be health in health science, public health science, digital health, health technology, implementation research, medicine, other health professionals or relevant social sciences.

We emphasize that you have:

  • strong methodological skills within one or more relevant research areas
  • Experience with collection, analysis and interpretation of research data

It is not a requirement that you have experience in all areas. We are looking for a methodically strong researcher with a solid professional core and want to work across methods and professionalities.

A practice-oriented researcher
An important part of the project is being conducted closely. You must therefore thrive to cooperate across professional and organizational environments and could move between research and clinical and municipal reality. As the project involves interviews, workshops and other direct cooperation with Danish citizens and practitioners, it is a requirement that you master Danish at high level, both orally and in writing. At the same time, you can communicate and publish in English.

Transition + is anchored at the Faculty of Health Sciences (SUND), Aalborg University, based on the Department of Medicine and Health Technology.

Om projektet
Sundhedsvæsenet er under forandring. Borgerne har imidlertid forskellige kompetencer for at navigere i sundhedsvæsenet. Modellen udvikles med universelt design som grundlæggende princip og i tæt samarbejde med borgere, sundhedsprofessionelle og organisatoriske aktører på tværs af sektorer.

Din rolle
Som postdoc bliver du en central del af projektets forskningsgruppe og får mulighed for at arbejde med hele forskningsprocessen fra kortlægning og udvikling til implementering, evaluering og publicering.

Dine opgaver vil blandt andet være at:

  • kortlægge eksisterende patient- og transitionsforløb samt praksis for identificering og håndtering af støttebehov
  • planlægge og gennemføre dataindsamling blandt borgere, patienter, sundhedsprofessionelle og organisatoriske aktører
  • anvende og kombinere relevante kvalitative og kvantitative metoder
  • bidrage til udvikling og operationalisering af Transition Support Model, herunder stratificering af støttebehov og udvikling af støtteprofiler
  • indgå i co-design med borgere, klinikere og kommunale medarbejdere
  • planlægge og gennemføre implementering af modellen i udvalgte patientforløb
  • bidrage til evaluering af modellens anvendelighed, implementering, betydning for borgerne og ressourcekonsekvenser
  • analysere og publicere resultater i internationale peer-reviewed tidsskrifter samt formidle til relevante praksispartnere.

Har en ph.d.-grad inden for et relevant område og erfaring med selvstændigt forskningsarbejde og videnskabelig publicering. TRANSITION+ er forankret på Det Sundhedsvidenskabelige Fakultet (SUND), Aalborg Universitet, med udgangspunkt i Institut for Medicin og Sundhedsteknologi.

Projektet gennemføres i et tværfagligt forskningsmiljø med samarbejde på tværs af relevante forskningsmiljøer på SUND, herunder Klinisk Institut (KI), og i tæt samarbejde med Mariagerfjord Kommune, som er central praksispartner i udvikling, afprøvning og evaluering af Transition Support Model.
